Case Manager- GPD-SITH Pompano Beach

General Description: Provides dedicated case management services to Veterans experiencing homelessness in a service intensive transitional residential housing program, focusing on an individualized service plan that yields long term housing stability outcomes. Case managers are expected to employ a strengths-based approach, which empowers and promotes the strengths and capabilities of Veterans.

 

Primary Job Responsibility & Duties: 

  1. Completes intake, assessment, service plans in coordination with client and within documentation standards set forth by the program contract, Federal Regulations and Guidelines, and United Way of Broward County/MISSION UNITED.
  2. Wholistically addresses the case management needs of the individual client.
  3. Provides field-based, strength-based case management and service coordination that includes housing stabilization, money management, community integration, employment linkages, benefits establishment, linkage to community providers for services needed to assist clients in reaching the housing stability plan goals.
  4. Develops or updates service plans with clients to address their changing needs and to facilitate referral to appropriate treatment or service providers or facilities.
  5. Provides ongoing case management support to assess progress and ensure case plan outcomes are met or changed, as needed.
  6. Utilize evidence-based practices in service delivery such as Motivational Interviewing, Harm Reduction, Critical Time Intervention and Housing First Practices.
  7. Maintains documentation and data entry standards as set forth by the program contract, Federal Regulations and Guidelines, and United Way of Broward County/MISSION UNITED, including HIPAA policies and procedures.

Other Job Duties:   

  • Conducts assessments of the individual’s needs, financial stability, work experience, medical and behavioral health history, and more.
  • Develops actionable client directed individualized case plans, including identified strengths, needs, risks, assets, etc. to develop short- and long-term goals and objectives related to housing stability and financial prosperity.
  • Identifies needed resources and services.
  • Ensures programmatic compliance.
  • Performs drug testing on site and is able to identify Veterans in need of treatment services, as appropriate.
  • Conducts daily check-ins of client’s living environment, needs, barriers, activities, etc.
  • Creates an environment where people can feel welcomed, comfortable, understood, and esteemed.
  • Documents all client encounters, utilizing the Homeless Management Information System (HMIS) and other client information systems, within 24 hours of meeting with the client.
  • Other Duties, as assigned.
  • Manages a caseload of 25-30 individuals at all times.
  • Advocates on behalf of clients for services, basic needs, and other related issues.
  • Composes or prepares correspondence, case notes, narrative and technical reports, notifications, and related documents using computer-based applications.
  • Conduct comprehensive, client-centered social work activities in accordance with best practices.
  • Conducts community and home visits (if needed) related to client needs; Provides follow-up and monitors service delivery and care environments.
  • Stays abreast of community resources and procedures for referring clients to appropriate resources.
  • Facilitates case consultation for client situations.
  • Establishes case plan goals and will review with clients at 30 day intervals.
  • Monitors client progress to ensure that clients are working towards a resolution of identified challenges to meet their housing stability goals.
  • When appropriate, ensures successful case closure, documentation, and transition.
  • Provides crisis prevention, intervention, and supportive counseling, as needed.
  • Performs other duties and assumes other responsibilities as assigned by supervisor.
  • Due to the leadership role United Way is called upon to play in the community, particularly during times of crisis, it is the expectation that all United Way staff will be fully engaged in the organization’s crisis plan and response efforts.

Supervises: This position does not have any supervisory responsibilities.

Education & Experience: 

  • Bachelor’s degree with major course work in social work, mental health, or other closely related field. At least two (2) years of experience in general interviewing practices and/or techniques, counseling, social case/service work (including crisis prevention and intervention),community resource referral, or closely related work with preferred experience working with Veterans and/or persons with mental health conditions.
  • Prior case management experience with knowledge of Veterans Administration related programming highly preferred.
  • Knowledge of evidence-based practices, outcome measurement, and quality management.
  • Experience working with low income, homeless population, Veterans, military families, and/or service members strongly preferred.
  • Ability to communicate effectively with a diverse audience.
  • Experience with Microsoft Office Suite to include MS Word, Excel, and PowerPoint.
  • Strong desire to help others in need. Must be self-motivated, compassionate, have a positive attitude and determined to advocate for the rights and needs and vulnerable people in our community.
