Homeless Veterans’ Reintegration Program (HVRP) Employment Case Manager

General Description: Provides dedicated employment-related case management along with service and resource coordination to veterans experiencing homelessness as determined eligible and appropriate for services according to criteria stated by the Department of Labor’s HVRP grant. 

Primary Job Responsibilities & Duties:

  1. Performs needs and comprehensive assessments; develops individualized employment plans (IEP) and resumes with clients to address their employment needs and other barriers that may impact employment; facilitates referrals to appropriate service providers or agencies.
  2. Ensures clients achieve IEP goals and sustain successful long-term employment by working directly with the local American Jobs Centers (AJC), employers, and other employment-readiness services; ensures the client is connected with trainings, certifications, and education opportunities to achieve their IEP goals. 
  3. Works hands-on with clients and employers from the initial application for employment, through the hiring process, and throughout the first year of the client’s employment to ensure the client is meeting and sustaining expectations of employment. 
  4. Maintains a physical and electronic client case file to ensure compliance with HVRP grant standards with a priority on documenting all correspondence, assessments, activities, outcomes, and expenditures for each client managed.  

Other Job Duties:

  • Ensures each client is eligible and appropriately suited for success under HVRP standards. 
  • Collaborates with case managers and other advocates working on behalf of the client to ensure a unified and holistic approach to service.
  • Coordinates and consistently documents in accordance with best practices and Council on Accreditation (COA) accreditation standards the clinical case management and psychosocial services. 
  • Conducts comprehensive, client-centered social work activities in accordance with best practices.
  • Conducts weekly follow-ups with the client and employer including community and home visits (if needed) related to client needs; Provides follow-up and monitors service delivery and employment  environments.
  • Stays abreast of community resources and procedures for referring clients to appropriate resources
  • Provides crisis prevention, intervention, and supportive counseling as needed. 
  • Performs other duties and assumes other responsibilities as assigned by supervisor.
  • Due to the leadership role United Way is called upon to play in the community, particularly during times of crisis, it is the expectation that all United Way staff will be fully engaged in the organization's crisis plan and response efforts.

Supervises:  This position has no supervisory responsibilities.

Education & Experience:

  • Bachelor’s degree in social work, mental health, human resources, or other closely related field.
  • At least two (2) years of experience in employment-related case management, human resources, or closely related work with preferred experience working with low income, veterans, military families, persons experiencing homelessness, substance use, or mental health conditions. 
  • Knowledge of evidence based best practices, individualized treatment planning, outcome measurement, and quality management. 
  • Ability to communicate effectively with a diverse audience of clients, case managers, and professionals. 
  • Veteran status preferred.
